McGillvray ejected after an engine fire on takeoff from Baden-Söllingen. This was his second CF-104 ejection. Aircraft landed in a cemetery. Board of Enquiry determined that the progressive engine failure resulted in eleven major warning lights in 7 minutes and 42 seconds.
CT-133 133315, CAF, c/n T33-315, ex 21315, to 133315 11.11.1970, w/o 07.04.1987 crashed mid-morning 7 April 1987 while flying a low-level navigation mission. While flying down a valley in the Eifel Hills near Koblenz the Canadian T-33 encountered a Belgian AF F-16 also flying a training mission. In accordance with NATO training regulations in force at the time, the T-33 initiated a 180 degree defensive turn to the left to acknowledge the "threat". During this maneuver the T-33 impacted rapidly rising, heavily forested terrain and was totally destroyed. There was no attempt by either pilot to eject.

In the busy airspace east of Liège the Mirage collided with Morane-Saulnier MS880B OO-NSD that had just taken off from Kiewit airfield near Hasselt. The Miragepilot, Kapt.vl.Philippe Delferrière, managed to save himself with his ejection seat, but the 3 occupants of the Morane were all killed. The Mirage crashed near Visé. (2 Wing - 2 Squadron)

Tornado GR.1 ZA605, RAF, c/n 134, w/o 10.12.1996 collided with Tornado ZA611 during formation after take off for night mission. ZA605 crashed onto defunct railway line after Flt Lt R.P. Lewis and Flt Lt A.M. Randall ejected. ZA611 made emergency landing at Marham with part of wing missing.
Tornado GR.1 ZD717, RAF, c/n 334, w/o 14.02.1991after being hit by two Iraqi launched SAMs during a laser guided bombing mission on Al Taqaddum airfield, Iraq. The pilot ejected, but the navigator was killed
Tornado GR.1 ZD791, RAF, c/n 400, w/o 17.01.1991 crashed near Ar Rumaylah Southwest Airfield -Iraq, Flak and/or SAM-16 AAM struck and exploded a Sidewinder missile on the aircraft during a low level ground attack mission over an Iraqi airfield. Both crew ejected but were taken prisoner and later released

Hawk T.1A XX186, RAF, c/n 312033, w/o 30.04.1998 the crew were forced to eject during air combat manoeuvres with another Hawk, over the Irish Sea 20 miles west of RAF Valley, Anglesey, due to a control restriction. They were both taken to Bangor hospital with slight injuries. The RAF Hawk fleet was grounded for a time as a result of this accident
Pilot later named as Flt Lt Nigel 'Buster' Edwards. Buster holds the rather unenviable record as being the last person to eject from a 74(F) Squadron aircraft. The accident occurred on 30th April 1998 after the flying controls jammed, the cause of which was a loose battery. Buster broke his left arm badly during the course of the ejection and spent 40 minutes floating in the Irish Sea prior to rescue.

Hawk T.1 XX241, RAF, c/n 312077, w/o 16.11.1987 This was the first "Red Arrows" Hawk crash where two of the Hawk T.1As (XX259 and XX241) had a wing tip collision which caused them to plunge to the ground in the village of Welton, near Lincoln, five miles east of Scampton, Lincs. during practice aerobatics.
Red 2 collided with Red 1 after the airbrake failed to operate during a display practice. Wreckage from XX259 fell onto houses at Monce Close in Welton, XX241 crashed into a field outside the village. Both pilots ejected and there were no casualties on the ground

The pilot of aircraft CF188747, using the call sign “Swift 32”, was part of a two-ship formation led by “Swift 31” for an air-to-ground training mission. The mission objective was to practice level deliveries of two Mark 83 inert bombs followed by two laser guided training rounds, simulating laser guided bombs, in the Cold Lake Air Weapons Range. The plan was to ingress to the target and drop weapons from 600 feet above ground level. To avoid simulated bomb fragmentation after dropping their bombs each pilot would fly a “breakaway manoeuver” comprising a steep turn through 90 degrees of heading change.
Following his Mark 83 drop, Swift 32 manoeuvred his aircraft in a manner that was suggestive of a pilot attempting to visually spot his weapon impact, losing over 200 ft of altitude in the process. Swift 32 then assumed tactical lead, with Swift 31 flying about 2 miles in trail of Swift 32 and lasing the target for Swift 32, who then dropped his laser guided training round. The ingress to the target was flown at approximately 500 feet above ground level.
Immediately after dropping his laser guided training round Swift 32 initiated a steep left turn, reaching a maximum left bank angle of 118 degrees while pulling approximately 5g. The aircraft nose began to pitch towards and then below the horizon, eventually reaching a nose-down pitch angle of minus 17 degrees and concurrently generating a large descent rate.
About 1.5 seconds before impact the aircraft began rolling right. The bank angle had reduced to approximately 30 degrees left and the pitch angle increased to approximately minus 10 degrees when ground impact occurred. Swift 32 made no radio calls during the turn, did not eject and was fatally injured when the aircraft struck the ground in a descending left turn.
The available evidence did not support a mechanical failure, bird strike or pilot incapacitation scenario. Therefore, it appears that the pilot was capable of controlling the aircraft but did not adequately monitor the aircraft’s flight path while manoeuvring in the low level environment, and allowed the aircraft to enter an overbank situation and the nose to drop well below the horizon. A recovery may have been attempted at the last second but there was not enough altitude available to safely recover the aircraft. While the reason for this lack of flight path monitoring is not knowable with any certainty, circumstantial evidence suggests that the pilot may have been distracted from the critical task of terrain clearance while attempting to spot his weapon impact.

Tornado GR.1 ZD710, RAF, c/n 326, w/o 14.09.1989, suffered multible bird strike after take-off from RAF Abingdon, damaging the engine. Pilot (Wing Commander Wright) and Navigator (Squadron Leader Lawton) ejected safely.
Tornado GR.1 ZD809, RAF, c/n 411, w/o 14.10.1999, the aircraft encountered problems en route to the Spadeadam bombing ranges, and in close proximity to Newcastle Airport airspace, was twice forced to abort the mission, and return to the coast in line with correct procedures before attempting again to set course for Spadeadam. The weather in the area had been worse than what had first been expected and this was a major contribution to the accident.
Tornado ZD809 was not fitted with any Ground Proximity Radar which would have indicated to the two man crew Flt Lt's Richard Ashley Wright pilot aged 30 and his navigator Sean Casabayo also aged 30 just how close to the ground they were at the critical moment when ZD809 had to pull a hard left manouver to avoid entering Newcastle Airport airspace again.
The pilot had no rudder control for the crucial closing seconds which is a normal occurrence in such a manouver for a few seconds. The pilot did not have seconds to spare and could not recover the aircraft without his rudder control.
The Tornado came out of the low cloud and crashed into the ground of a ploughed field near a wood. The two crew sadly did not eject and were killed instantly. Wreckage was blown over a wide area and much of it fell into treetops in the nearby wood.
The BOI (Board of Investigation) inquiry concluded that pilot error and poor crew resource management were the primary cause of the accident. However, a ten seater commercial aircraft was seen flying very low in the area just before the Tornado crashed and at least one small piece of wreckage found in the ploughed field some three weeks later and viewed by an ex-Tornado airframe technician via email said it could not have come from a Tornado but must have been from an other aircraft?
