Hawk T.1 XX182 RAF, c/n 312029, d/d 04.11.1977, w/o 14.06.1989 During a three aircraft rejoin manoeuvre the No 2 aircraft (XX182) collided at 10,000 feet over Borth, Dyfed with the No 3 aircraft (XX291). One aircraft crashed near Machynlleth, the other near Clarach, Powys.
The pilot of Hawk XX182 was Lt. M. H. Seymour, who ejected safely at FL 100 and 300 knots. The pilot of Hawk XX291 was named as Pilot Officer Simon Mark Tompkins, who failed to eject, and was killed. Hawk XX291 came down 6 km SE of Aberystwyth

Phantom FGR.2 XT908 RAF, c/n 2684, d/d 20.08.1968, w/o 09.01.1989 The sortie was planned as an air combat sortie between two Phantom FGR2's of 228 OCU from RAF Leuchars. XT908 was a trainer variant but had the control column in the rear cockpit removed.
The first intercept went as planned but in the climb back to 10,000ft, the pilot of XT908 (Sqn Ldr Nelson) suddenly informed his navigator (Flt Lt Moulds) that he was feeling unwell. The navigator then informed the rest of the formation of the emergency and provided the pilot with a steer towards base.
Shortly afterwards the pilot's level of consciousness rapidly decreased and although he was responding to commands, they were slow. The aircraft then began a decent and despite repeated requests, the aircraft descended. The navigator the shouted "Pull Up" and the pilot responded in an agitated manner and selected full dry power (no re-heat) and 10 degrees nose up. After this the pilot failed to respond to any requests.
The navigator selected full re-heat and as the aircraft passed 16,000ft, it departed from controlled flight and entered a spin. As there was no control column in the rear, the navigator ejected after being advised by the other aircraft not to leave his ejection too late and after informing the pilot.
The aircraft crashed into the sea and no trace of the pilot was ever found. Location of crash cited as being 56'30.00"North, 0' 20.00" West
This was the second time that this pilot had became unwell at the controls of a Phantom but on the other occasion he managed to land safely but one wonders why he was still allowed to fly?

The damage was such that it was uneconomical to repair. Aircraft officially struck off charge as Cat.5(G/I) on 11/03/1986: to ground instructional use as RAF Leuchars as 8913M. Finally scrapped in April 1992

The West Coast railway line and the adjacent A6 road were both closed while debris was recovered

It ran across the grass, through a copse of trees which are planted to the left of Runway 05, which ripped the wings and elevators from the aircraft. At the this time the crew had already ejected but the aircraft was still on take off power.
The fuselage hurtled towards the waiting line of traffic waiting at the traffic lights to cross the runway, got airborne as the sloping ground dropped away crossed an access road where the nose caught the kerb and rolled the fuselage which came to rest which an angle iron fence post sliced into the right hand side of the cockpit. The Construction crew building the rolling deck facility at the time had a grandstand view. The student test pilot received spinal injyuries while the instructor received minor injuries
After the Board of Enquiry was concluded, the remains were passed to the then fledgling Boscombe Down Aviation Collection (BDAC).
